Jack Pelphrey, age 83, of Flat Gap, Kentucky, passed away Friday, October 8, 2021 at the Community Hospice Center in Ashland, Kentucky.

Jack was born August 17, 1938 in Johnson County, Kentucky, son of the late Brooks and Swannie Lemaster Pelphrey. In addition to his parents, he is preceded in death by three brothers, Bob Pelphrey, James Pelphrey, and Kelsey Pelphrey and two sisters, Freda Murray Wooten and June Stapleton.

He is survived by his wife, Phyllis McKenzie Pelphrey; two daughters, Jacqueline Cloma of Flat Gap, Kentucky and Shawnann Crider of Staffordsville, Kentucky; two grandchildren, Shelby (Ethan) Ritz and Trenton Crider; two brothers, Kenny Pelphrey of Oil Springs, Kentucky and John G, Pelphrey of Stambaugh, Kentucky; and one sister, Edna Grace Caudill of Stambaugh, Kentucky.

Funeral services will be conducted 2:00 p.m. Saturday, October 16, 2021 in the Jones-Preston Funeral Chapel with Sammie Reed and David Young officiating. Burial will follow at Dixon Cemetery, Staffordsville, Kentucky. Friends may visit the Jones-Preston Funeral Home from 5:00 p.m. until 8:00 p.m. Friday, October 15, 2021 and 12:00 Noon until time of service at 2:00 p.m. Saturday, October 16, 2021.
